If you’re living with diabetes, heart disease, or another chronic condition, you could qualify for a specialized Medicare plan to help support your specific health needs.
Chronic Condition Special Needs Plans are Medicare Advantage plans that provide extra resources and tailored benefits to people with specific chronic conditions.
Learn more about C-SNP and Dual Eligible Special Needs Plans (D-SNP) in our on-demand webinar: Making Medicare Work for You: Chronic and Dual Plan Focus.
If you’re new to Medicare, you can enroll during your Initial Enrollment Period, which starts three months before your 65th birthday month and ends three months after. You can also sign up during the Annual Enrollment Period, which runs from October 15 to December 7 each year. If you’ve recently moved or received a diagnosis of a specific chronic condition, you may qualify for a Special Enrollment Period, allowing you to enroll in a C-SNP plan right away.
We can help you determine your eligibility and ensure you are maximizing all the benefits and services available to you. If you or someone you know may qualify for a C-SNP plan, our knowledgeable advisors can help you find the right coverage for your needs and budget. We're passionate about helping people just like you, and we genuinely care.
If you have both Medicare and Medicaid, you may qualify for a Dual Eligible Special Needs Plan (D-SNP), a type of Medicare Advantage plan designed to support people with limited income and resources.
If you don't qualify for C-SNP or D-SNP, you can still enroll in a Medicare Advantage plan that includes coverage for doctor visits, hospital care, and prescription drugs—all in one plan.
We’ll look into all available plan options, give you the tools and resources you need to make a confident decision about your health plan, and handle the application and enrollment process―all at no cost. Once your plan enrollment is complete, we’ll continue to provide support and guidance to you for life.
