If you’re living with diabetes, heart disease, or another chronic condition, you could qualify for a specialized Medicare plan to help support your specific health needs.

What are C-SNP plans?

Chronic Condition Special Needs Plans are Medicare Advantage plans that provide extra resources and tailored benefits to people with specific chronic conditions.

Who qualifies?

  • If you have one or more of the 15 chronic conditions recognized by Medicare (including over 40 subcategories), you may be eligible. Doctor approval is required.
  • Conditions include cardiovascular disorders, chronic heart failure, diabetes mellitus, and more. See full list of specific chronic conditions as defined by CMS here.
  • C-SNP availability and the specific conditions they cover depend on where you live.

Enrollment Periods for C-SNPs

If you’re new to Medicare, you can enroll during your Initial Enrollment Period, which starts three months before your 65th birthday month and ends three months after. You can also sign up during the Annual Enrollment Period, which runs from October 15 to December 7 each year. If you’ve recently moved or received a diagnosis of a specific chronic condition, you may qualify for a Special Enrollment Period, allowing you to enroll in a C-SNP plan right away.

Not eligible for a C-SNP? You still have options.

If you have both Medicare and Medicaid, you may qualify for a Dual Eligible Special Needs Plan (D-SNP), a type of Medicare Advantage plan designed to support people with limited income and resources.

If you don't qualify for C-SNP or D-SNP, you can still enroll in a Medicare Advantage plan that includes coverage for doctor visits, hospital care, and prescription drugs—all in one plan.
